Client Care Coordinator
Joanne is a compassionate registered nurse with over thirty years of experience in healthcare, most recently serving as a case manager in the emergency department. With her extensive background, Joanne deeply understands the complexities involved in senior housing, home care options, and care coordination. She has seen, firsthand, how overwhelming and confusing the system can be for families, often lacking the necessary guidance or follow-up regarding available community services. These experiences have only strengthened her dedication to ensuring that our elders receive the superior care they truly deserve, especially through clear and timely communication about vital resources for families.
Recognizing that many families lead busy lives filled with work and daily responsibilities, Joanne is keenly aware of the challenges involved in finding the right living arrangements for senior loved ones. She is passionate about supporting individuals and families through this process, assisting them in navigating the various solutions available. Joanne is wholeheartedly committed to advocating for you and your loved ones, guiding you with empathy and understanding as you make informed decisions during this important journey.

After a twenty-five year carer as a successful entrepreneur, George with Joanne felt a deep calling to launch Compassionate Eldercare, driven by his personal journey of navigating the challenges of caring for aging relatives. He experienced the frustration and heartache of not having enough resources to provide the level of care they truly deserved. Through this experience, George saw how vulnerable families can be, often at the mercy of an overworked and understaffed healthcare system, as well as private providers who may not prioritize their unique needs.
This understanding fueled his passion to dedicate his life to helping families with compassion, fairness, and honesty. George is wholeheartedly committed to ensuring that every family feels empowered and secure with personalized care plans and living options that Compassionate Eldercare provides, reflecting the love and respect their loved ones deserve.

Eligibility and enrollment assistance for long-term care Medicaid. We offer a cost-effective solution to this time consuming task. The process can be challenging however, our dedicated assistance will simplify the course, ensuring you understand each step and feel confident along the way. Let us make applying for Medicaid a stress-free experience for you.
We begin by assessing the level of care your family member requires based on their diagnosis and individual needs. For seniors needing care, we will complete a medical assessment to ensure we recommend options that address both their medical and personal care needs. After our evaluation, we will set up tours of the facilities that best suit your requirements.
PRI (Patient Review Instrument) and Screen assessments are used in New York State to evaluate an individual's qualification for long-term care and to identify the appropriate level of care they require. These assessments are mandatory and valid for 90 days. They must be completed by a certified Registered Nurse.
